No walk-ins. Space is limited and you must have an appointment. Please bring your vaccine card and your insurance card. Masks are required. We are offering Moderna and Pfizer for those ages 18 and older.
Where: 100 Stafford Lane in Delta, CO inside the Grand Mesa Medical Building (Please use the Urology Clinic Entrance)
